Please read this document carefully before signing.1. Assumption of RiskI understand that dance fitness involves intense physical activity, including rapid movements, jumping, stretching, and aerobic conditioning. I acknowledge that these activities carry inherent risks of injury, including but not limited to muscle strains, sprains, falls, and cardiac events. I knowingly and freely assume all such risks.2. Medical Fitness and Safety I represent that I am in good physical condition and have no medical, physical, or psychological condition that would prevent my safe participation in dance fitness classes. I agree to disclose any injuries or health concerns to the instructor prior to class. I assume full responsibility for monitoring my own physical limits and will stop immediately if I feel dizzy, faint, or experience pain.3. Release and Waiver of Liability. I, on behalf of myself, my personal representatives, heirs, and assigns, hereby release, discharge, and hold harmless [Danc2fitWithHannah], its owners, instructors, employees, and agents from any and all liability, claims, or demands for personal injury, property damage, or wrongful death arising out of my participation in these classes.4. Photo and Video ReleaseI grant [Dance2FitWithHannah] permission to use photographs, video recordings, or audio clips of me taken during classes for promotional, marketing, or social media purposes. By agreeing below, I certify that I am at least 18 years of age (or have parental/guardian consent) and that I have read and fully understand this waiver and release of liability.
