NYSDOH Incident Reporting 2026: From First Notification to Five-Day Investigation NYSDOH has released its newly updated 2026 Nursing Home Incident Reporting Manual. Is your facility ready? New York nursing homes are working under updated NYSDOH guidance addressing what must be reported, when the reporting clock begins, who is responsible, what happens during the investigation, and what must be included in the five-business-day investigative summary. The revised manual also brings together current state and federal reporting requirements and provides updated reportability scenarios for nursing homes. Join Moneague Care Compliance for a live, in-person educational program designed for nursing home administrators, nursing leadership, risk managers, nurses, investigators, operators, and other team members responsible for incident recognition, reporting, investigation, and follow-up. This is not just a review of the manual. Participants will work through realistic nursing-home scenarios and learn how to move an incident from first notification through the five-day investigation while avoiding common reporting and documentation gaps. Your own educational application identifies the current practice gap as inconsistent recognition of reportable events, incorrect application of reporting deadlines, unclear responsibility, and incomplete five-day investigations. Come prepared to test what you know, work through scenarios, and leave with practical tools you can take back to your facility. Continuing Education Licensed Nursing Home Administrators: This educational offering has been submitted to the National Continuing Education Review Service (NCERS) of the National Association of Long Term Care Administrator Boards (NAB) for 3.0 continuing education participant hours. Approval pending. State licensing boards have final authority regarding acceptance of continuing education for license renewal.
