Fresno C2C Partnership Meeting

Help build a Community Health Network that serves everyone. Fresno C2C Partnership Meeting If you are part of a school district, a community school, or a community-based organization, we need you to connect with the Community Health Network and share your perspective as we grow this initiative! Join us on Friday, December 11th, to learn how Community Health Workers (CHWs) are changing the way our children and families access and stay connected to the services and supports they need. This meeting offers a unique opportunity to learn why Parlier and Central School Districts are training and certifying existing staff members to become CHWs and how this investment of time and money is paying off. Our panel will include a Community Health Worker, a parent, and a school district leader. Expanding the number of Community Health Workers is a key strategy in our work to transform our community health system. CHWs are trusted members of their communities who are culturally competent and uniquely equipped to meet families where they are, connect them to appropriate services, and do the important follow-up. This approach is foundational in building a Community Health Network that truly supports our local families. We will also have dedicated time for small table discussion and individual networking. Come and share your perspective and connect with others who share your passion to make a difference!
