Turning 18 is an important transition for youth with disabilities, join us to talk about planning for the transition into adulthood Turning 18 is an important transition for youth with disabilities, as eligibility, services, decision-making, and support systems can change significantly at the age of majority. This presentation provides a practical “before and after 18” look at what families and young adults need to know and plan for. Participants will explore changes to programs and benefits, eligibility requirements, assessments, applications, decision-making and consent, and available adult services and supports. The presentation will also highlight what can be started before age 18, what may need to be reassessed after 18, and how families can build a transition plan that supports continuity of care and greater independence. Please also note: anyone who needs specialized behaviour/medical support will require their own support staff to attend the group with them.
