ADHD or Something Else? Ten Decisions Before You Reach for a Stimulant

3 NBCC approved CE hours in Birmingham, Alabama on October 29th, 2026 from 11:30-2:30pm Attention problems are among the most common complaints encountered in psychiatric and primary care practice, but impaired concentration does not necessarily indicate attention-deficit/hyperactivity disorder (ADHD). Sleep disorders, mood and anxiety syndromes, hormonal changes, substance use, medication effects, medical illness, and neurologic conditions can all produce symptoms that resemble or exacerbate ADHD. This case-based, clinically practical presentation examines ten diagnostic decisions clinicians should consider before diagnosing ADHD or initiating stimulant treatment. Particular emphasis will be placed on distinguishing lifelong neurodevelopmental ADHD from later-onset cognitive complaints, identifying common psychiatric and medical contributors to inattention, and determining when the available evidence is sufficiently consistent with ADHD to proceed with treatment. The session is designed to improve diagnostic reasoning while reducing both under-recognition of legitimate ADHD and inappropriate attribution of nonspecific cognitive symptoms to ADHD Learning Objectives At the conclusion of this activity, participants should be able to: • Differentiate the developmental history and clinical presentation of ADHD from new-onset or secondary inattention arising from psychiatric, medical, sleep-related, hormonal, neurologic, or substance-related conditions. • Identify common psychiatric and medical disorders, medications, and substances that can cause or worsen impaired attention, executive dysfunction, and subjective cognitive complaints. • Apply a structured differential-diagnostic approach to patients presenting with possible adult ADHD, including assessment of symptom chronology, functional impairment, comorbidity, and potential reversible contributors. • Recognize clinical features suggesting sleep disorders, mood disorders, anxiety and trauma syndromes, perimenopausal or menopausal cognitive changes, substance effects, medication effects, and systemic medical illness as contributors to attentional symptoms. • Determine when the diagnostic evidence for ADHD is sufficiently established to support stimulant treatment and when additional assessment or treatment of competing conditions should occur first.
