Soulful Sages: Learn how to name your health care agent and complete a free California advance directive. For seniors, caregivers, and all a Who speaks for you if you can’t speak for yourself? Many people say, “I don’t want to live that way.” Few write down what “that way” means. This free 90-minute workshop helps you change that. You’ll learn what a basic advance directive covers and how to fill out the free California form. You’ll also learn how to sign it so it’s legal. You’ll leave with a take-home workbook and a script for asking someone to be your health care agent. What you’ll learn • What a basic advance directive covers • How to choose your health care agent, and how to ask them • Treatment choices to talk through: CPR, breathing machines, feeding tubes, and more • Palliative care, hospice, and comfort care • How a death doula supports you and your family • How to complete and sign the free California form Who should attend Seniors, caregivers, and all adults 18 and older. No prior planning needed. Your presenters Nicole Soares, RScP is the founder of EcoLove Transitions and co-leader of Soulful Sages. She is an End-of-Life Strategist, Advance Care Planning Guide, Certified Death Doula, and Licensed California Funeral Director (FD# 5082).
